Hindered muscle relaxation in spasticity: experimental evidence suggesting a possible pathophysiological mechanism

Abstract
In ten spastic patients and in an equal number of healthy controls, the relaxation phase occurring at the end of an isotonic voluntary contraction has been studied on soleus muscle using EMG and H-reflex methods. In the spastic group, the duration of motor unit-decruitment was consistently prolonged (from two to six times the control values). Moreover, the decrease in the excitability of the H-reflex arc, which normally accompanies the end of muscle contraction was delayed in time, reduced in amplitude or in some cases even absent. As a rule, patients in whom such release-associated inhibition (RAI) was lacking or severely reduced exhibited the longest motor unitdecruitment times, due to the interference of sustained clonic sequences. It is proposed that a lowered effectiveness of RAI might explain the clonic activity which frequently hinders voluntary muscle relaxation in spasticity. In dieci pazienti spastici e in un ugual numero di controlli sani la fase di rilasciamento muscolare che pone fone ad una contrazione volontaria isotonica è stata studiata sul muscolo soleo con tecniche reflesso logische. L'analisi dell'EMG ha dimostrato che nei pazienti spastici il tempo di de-reclutamento delle unità motorie è significativamente prolungato (da due a sei volte i valori di controllo). Inoltre, l'inibizione del riflesso H che normalmente si accompagna al rilasciamento muscolare è apparsa ritardata nel tempo, ridotta in ampiezza o, in alcuni casi, addirittura assente. Di regola, i pazienti in cui tale effetto inibitorio mancava o era notevolmente ridotto, mostravano la massima difficoltà a rilasciare volontariamente il muscolo in esame, a causa della comparsa di sequenze cloniche sostenute nel tempo. Questi risultati suggeriscono che una ridotta funzionalità dei meccanismi inibitori (presinaptici) normalmente operanti al termine di una contrazione volontaria sia alla base dell'attività clonica che assai spesso ostacola il rilasciamento muscolare nei pazienti spastici.This publication has 17 references indexed in Scilit:
