Endoscopic resection of mucosal cancer of the esophagus

Abstract
The indications for curative endoscopic resection of esophageal cancer are as follows: (1) mucosal cancer apart from gross invasion to the muscularis mucosae; (2) less than 2 cm × 2 cm in size or less than a third circumference of the esophagus; (3) absence of nodal involvement. At present two procedures are applied for esophageal mucosal resection in our department. One involves capturing the mucosal lesion with grasping forceps through opened snare forceps and snaring it. The other is endoscopic mucosal resection under negative pressure at the tip of the endoscope. A small cap is attached to the distal part of the endoscope. The snare forceps through the working channel of the endoscope are opened in the cap and the protruding mucosa is snared into the cap by the suction mechanism of the endoscope. Endoscopic resection was performed in 17 cases. All patients had mucosal cancer. In all instances complete response was obtained locally. There was no post-operative complication. The longest period of follow-up is 3 years and 8 months and no recurrence has been observed in any case.Keywords
