Suicides on the increase in psychiatric clinics. Facts, causes, prevention

Abstract
In den Psychiatrischen Kliniken von fünf anderen Ländern und in der eigenen Klinik haben die Suizide in den letzten Jahrzehnten signifikant und um ein Mehrfaches stärker zugenommen als die Suizide in der Gesamtbevölkerung. Wahrscheinlich sind hierfür nicht in erster Linie änderungen in der soziographischen und diagnostischen Zusammensetzung der aufgenommenen Kranken verantwortlich, sondern Fortschritte in der liberalen Krankenbehandlung und in der Rehabilitation. Suizidpräventiv ist im Auge zu behalten, dass eine Minderzahl von Kranken nicht erträgt, was der grossen Mehrzahl hilft. Dans les cliniques psychiatriques de cinq pays et dans la nÔtre, les suicides ont significativement augmenté depuis environ cinquante ans, et plusieurs fois plus que les suicides dans la population générale. En toute probabilité ce ne sont pas en premier lieu des changements dans les caractéristiques sociographiques ou nosographiques de nos malades qui en sont responsables, mais plutÔt un traitement hospitalier plus libéral et plus orienté vers la réhabilitation. Du point de vue préventif, il faut se rappeler qu'une minorité des hospitalisés supporte mal ce qui est avantageux pour la grande majorité. In the psychiatric clinics of five countries and in ours as well, suicides have significantly increased since the twenties, and many times more than in the general population. This trend is probably not due to change in the sociographic and diagnostic composition of admitted patients but to more liberal hospital treatment and more active rehabilitation. In order to prevent suicide in the clinic, it has to be kept in mind that what proves to be an asset for the great majority of patients may be detrimental for a minority.
