A 23-year-old gravida 3, para 2, was dead on arrival with the breech and trunk of a 650-g fetus protruding from the introitus. After delivery of the head, about 1/4 of the placenta was found still firmly implanted; it was removed manually. A small recent puncture wound was detected halfway between symphysis and umbilicus, and the patient was quite obviously exsanguinated.
