Fibrinolysis vs Disseminated Intravascular Coagulation

Abstract
To the Editor: In the May 15 issue of the Journal, Charytan and Purtilo reported glomerular capillary thrombosis as a complication of epsilon-amino caproic acid (EACA — Amicar) therapy in a man with severe fibrinolysis.1 The authors concluded that the fibrinolysis was primary and that the complication reported was unique. We believe their data could be interpreted as showing that the excessive fibrinolysis was a reaction to a concomitant process, disseminated intravascular coagulation (DIC), and that thrombosis might have been averted with simultaneous heparin therapy.It is difficult to exclude the presence of DIC when severe fibrinolysis is manifest. . . .
