To the Editor: The patient with functional mitral regurgitation discussed in the September 5 Case Records of the Massachusetts General Hospital is very similar to a patient we have studied.A 57-year-old engineer with no prior cardiac disease or symptoms had had for eight years intermittent interscapular back pain, occurring in the supine and relieved when he assumed an upright position. He entered the Harbor General Hospital on August 1, 1968, with severe respiratory distress and in profound pulmonary edema.Upon recovery, an apical holosystolic murmur of Grade 3 intensity and a soft third heart sound were heard.
