Abstract
Miller and his colleagues (July 13 issue)1 report on the use of quinidine and exchange transfusion for the treatment of severe malaria in the United States. Although they did not conduct a randomized, prospective study of the use of exchange transfusions, the authors conclude that exchange transfusions are a valuable adjunct to chemotherapy. We would like to share data from a randomized, prospective study of exchange-transfusion therapy in Harare, Zimbabwe, and raise certain questions about the use of exchange transfusions for severe malaria.
