Zur Problematik der Milzautotransplantation -Vergleichende tierexperimentelle Studie des Implantationsortes

Abstract
Iatrogenically induced local splenosis seems to be a method for the partial preservation of splenic functions with appealing surgical simplicity. Pathophysiological aspects, such as flow direction of splenic venous blood, the ectopic occurrence of potential splenic affections, and the formation of pseudoabscesses as well as the preconditions for nidation of the autografts have not been sufficiently investigated so far. Sixty Wistar-rats, divided into four groups according to the surgical procedure performed (sites of implantation: omentum majus, liver, retroperitoneal space, pedicular omental flap), were subjected to macroscopical and histological studies with regard to the nidation of splenic autografts. Based on the results obtained and considering the above mentioned criteria, a pedicular omental wrap technique with or without retroperitoneal shifting and autograft fixation with fibrin glueing are suggested as the most preferable procedure. Die iatrogene lokalisierte Splenosis stellt eine chirurgisch scheinbar bestechend einfache Möglichkeit zur Erhaltung von Teilfunktionen der Milz dar. Pathophysiologische Gesichtspunkte wie Drainagerichtung des Milzvenenblutes, Ektopie potentieller Milzerkrankungen und Pseudoabszeßbildungen finden ebenso wie die Voraussetzungen zur Implantatnidation bisher nur ungenügende Beachtung. An 60 Wistar-Ratten, aufgeteilt in 4 Gruppen entsprechend dem chirurgischen Vorgehen (Implantationsort: Omentum majus, Leber, Retroperitoneum, gestielter Omentumlappen), wurde makroskopisch und histologisch das Angehen der Implantate untersucht. Aufgrund der Ergebnisse und unter Berücksichtigung der o. g. Kriterien wird die Milzpartikelimplantation in einen gestielten Netzlappen mit oder ohne retroperitoneale Verlagerung in das Milzbett sowie die Partikelfixation mit Fibrinkleber vorgeschlagen.
