Abstract
THE INCIDENCE and prevalence of kidney failure in the United States have increased substantially in the past 20 years, with the number of patients treated for kidney failure with dialysis or transplantation reaching more than 300 000 by the end of 1997.1 The number of individuals with mild kidney dysfunction, however, is much larger. A recent study2 estimated that as many as 5.6 million adults in the United States, representing 3% of the population, have early renal insufficiency (serum creatinine levels ≥1.6 mg/dL in men and ≥1.4 mg/dL in women).
