Abstract
A brittle diabetic who developed transient episodes of left hemiparesis associated with mild hypoglycemia is reported. Four vessel angiography revealed a right internal carotid artery stensosis which was surgically removed. Subsequently, during hypoglycemia, the focal neurological deficit did not occur. The necessity for investigating all diabetics who develop a focal neurological deficit with hypoglycemia to determine if there is a focal vascular abnormality is emphasized.
