Effect of Chronic Aromatase Inhibition by Δ1 -Testolactone on Pituitary-Gonadal Function in Oligozoospermic Men

Abstract
Summary: Aromatase inhibition by Δ1 -testolactone (Teslac®, 500 mg twice daily) for 6 months in 9 patients with idiopathic oligozoospermia lowered the levels of serum estradiol (E2) and thereby sex hormone binding globulin (SHBG) (rs= +0.40, p < 0.025) to values -35 and -25%, respectively, below the pretreatment values (P < 0.001 and < 0.005). The E2 decrease was accompanied by a temporary increase (+50%) in the levels of follicle stimulating hormone (FSH), not of luteinizing hormone (LH), and of 17 α-hydroxyprogesterone (17α-OHP), but less of testosterone (T) (+30%), which led to a transient rise in the 17α-OHP/T ratio. The T/E2 ratio and “free T” index (T/SHBG) almost doubled until the end of the treatment period. During Δ1 -testolactone treatment the mean sperm density gradually rose from 8.1 ±1.3 (SEM) before to 21.3 ± 6.7 times 106/ml after 6 months (P < 0.01), whereas the total sperm count almost threefold increased (P < 0.05). Sperm concentrations exceeding 20 times 106/ml were achieved in 4 of the 9 patients. Two of these patients' wives became pregnant. Although the data point to a pivotal role of estrogens in the pathogenesis of the spermatogenic lesion in some patients with idiopathic oligozoospermia, the lack of a benificial effect of estrogen lowering in others points to a multicausal nature of the disease entity. Zusammenfassung: Die Blockierung der Aromatase durch Δ1-Testolacton (Teslac®, 2 times 500 mg/die über 6 Monate) bei 9 Patienten mit idiopathischer Oligozoospermie führte zu einer Erniedrigung des Oestradiol (E2 -)-Serumspiegels urn 35% des Ausgangswertes (P < 0.001). Als Folge davon fiel auch das “sex-hormone-binding-globuline” (SHBG) urn 25% des prätherapeutischen Wertes ab (P < 0.005). Der E2-Abfall ging mit einem vorüber-gehenden Anstieg (50%) des follikelstimulierenden Hormons (FSH) einher, während die Serumspiegel des luteinisierenden Hormons (LH) und des 17a-Hydroxyprogesterons (17a-OHP) unverändert blieben. Der Testosteron (T)-Spiegel erhöhte sich um 30%, was zu einem zeitweisen Anstieg des 17α-OHP/T-Verhältnisses führte. Das T/E2 -Verhältnis und der “free T index” (T/SHBG) verdoppelten sich beinah bis zum Ende der Behandlungsperiode. Während der 6-monatigen Behandlung mit Δ1 -Testolacton stieg die Spermatozoenkonzen-tration stufenweise von 8, 1 auf 21,3 times 106/ ml an (P < 0.01). Die Spermatozoengesamt-menge erhöhte sich urn das Dreifache (P < 0.05). Spermazozoenkonzentrationen über 20 times 106/ml wurden bei 4 oder 9 Patienten erzielt. Die Ehefrauen von 2 dieser Patienten wurden schwanger. Obwohl die Ergebnisse auf eine zentrale Rolle der Oestrogene in der Pathogenese sper-matogener Störungen bei Patienten mit idiopathischer Oligozoospermie hinweisen, spricht das Versagen einer antioestrogenen Therapie bei einem Teil dieser Patienten eher für ein multikausales Geschehen.Keywords
