Comparison of orthodromic and antidromic sensory nerve conduction velocity measurements in the carpal tunnel syndrome

Abstract
Orthodromic and antidromic nerve conduction velocities were determined in sensory median nerve fibres from digit to palm and from palm to wrist in patients with the carpal tunnel syndrome. In a large number of subjects the carpal tunnel syndrome could be detected only when the palm-to-wrist segment was investigated. Comparison of the results of the corresponding orthodromic and antidromic nerve conduction velocity measurements along the different segments revealed no significant disadvantages for the antidromic method. This technique should be given preference to the orthodromic method since it is easier. Orthodrome und antidrome Nervenleitgeschwindigkeiten wurden im sensiblen N. medianus in den Segmenten Finger III-Handgelenk, Finger III-Hohlhand und Hohlhand-Handgelenk bei Patienten mit einem Karpaltunnelsyndrom untersucht. Bei einer großen Zahl von Patienten konnte ein Karpaltunnelsyndrom nur durch eine gezielte Untersuchung des Segmentes Hohlhand-Handgelenk nachgewiesen werden. Die Veränderungen antidromer und orthodromer Leitgeschwindigkeiten waren in den einzelnen Segmenten nahezu identisch, so daß bei der neurographischen Untersuchung eines Karpaltunnelsyndroms die Messung der antidromen Nervenleitgeschwindigkeiten allein ausreichend für die Diagnosestellung ist.This publication has 12 references indexed in Scilit:
