BLASTOMYCOSIS MENINGITIS: REPORT OF A CASE SUCCESSFULLY TREATED WITH AMPHOTERICIN B

Abstract
Involvement of the meninges with the organism Blastomyces dermatitidis, though rare, has been uniformly fatal. Treatment with amphotericin B. (Fungizone for infusion - Squibb; an antifungal antibiotic derived from an as yet unidentified species of Streptomyces) was given by slow (4-6 hours) intravenous infusion. Dosage was started at 25 mg/day dissolved in 1,000 cc of 5% D/W and gradually increased to 75 mg/day (1.25 mgAg) according to tolerance. Total dosage was 2.65 g of amphotericin B over a 57-day-period. The patient became asymptomatic after the first 400 mg of the drug was given. The spinal fluid studies improved more slowly, but steadily to normal. Now, 7 months after discharge, the patient remains completely asymptomatic and is on no medication.
