Effects of diltiazem on developing blood pressure and accompanying cardiac and vascular hypertrophy in SHR.

Abstract
高血圧症に伴う心肥大および血管肥厚に対するCa拮抗薬の影響を検討するために, SHRに4週齢より12週間に亘って, diltiazem 30ないし60mg/kgを1日2回連続経口投与した. diltiazem処置により, SHRの血圧上昇は有意に抑制されたが,心拍数は対照群と差を認めなかった.また,左心室重量は対照群に比べて有意に減少した(対照群; 268.2±5.5mg/100g体重, diltiazem 60mg/kg処置群; 248.0±3.7mg/100g体重, P<0.05).さらに,腹部大動脈のCa含量は有意に減少した(対照群: 14.56±1.05mmole/mg dry weight, diltiazem 60mg/kg処置群: 9.96±0.49mmole/mg dry weight; P<0.05).一方, diltiazem 60mg/kgをSHRに一回経口投与すると平均血圧は投与25~35分後に約45mmHg低下し,心拍数は投与35~60分後に約70beats/min減少した.同条件下に採血,測定した血漿レニン活性は対照群に比べて有意に上昇したが(対照群; 5.65±0.97ng/ml/hr, diltiazem 60mg/kg投与群; 11.5±2.22ng/ml/hr, P<0.05),血漿アルドステロン濃度は変化しなかった(対照群; 532.8±79.0pg/ml, diltiazem 60mg/kg投与群; 556.5±75.9pg/ml).さらに,麻酔下のSprague Dawley系ラットにdiltiazem 60mg/kgを胃内投与すると,降圧作用とともに,心収縮能の増加が認められた.以上の結果, diltiazemは抗高血圧作用とともに,心肥大抑制作用や血管壁Ca含量減少作用を有することが示唆された.This publication has 12 references indexed in Scilit:
