Abstract
ALTHOUGH 50 years have passed since the discovery of insulin, large gaps remain in the understanding of diabetes mellitus. The lack of knowledge is particularly evident when one considers the small-blood-vessel lesions of this disease, which, when affecting the kidney or the eye, represent a serious threat to the life and well-being of the patient and a great challenge to the clinician.Knowledge about the pathogenesis of diabetic microangiopathy is essential to provide a basis for rational treatment and prevention and to permit a sober evaluation of whether careful replacement therapy with insulin can achieve these desired results.
