Salvage surgery for recurrence after breast conservation

Abstract
Breast conservation surgery and radiation therapy are chosen with increasing frequency in breast cancer management. In addition to the risk of developing a contralateral malignancy, these women are at risk for local recurrence or a new primary lesion in the conserved breast. Most of these ipsilateral recurrences can be treated successfully with salvage mastectomy. The prognosis with salvage mastectomy depends on several factors, including the method of diagnosis of the recurrent tumor, the size of the recurrence, the extent of breast involvement at recurrence, the time interval from initial surgery to recurrence, the involvement of axillary nodes at the initial diagnosis and at the time of recurrence, and the histopathology of the recurrent tumor. On préconise de plus en plus souvent une chirugie conservatrice associée à la radiothérapie en matière de cancer du sein. En plus du risque de développer un cancer de l'autre côté, ces femmes sont à risque de récidive locale ou de faire un deuxième cancer dans le sein conservé. La plupart de ces récidives, cependant, peuvent être traitées convenablement par une mastectomie de sauvetage. En revanche, le pronostic de ces mastectomies de sauvetage dépend de plusieurs facteurs, y compris la méthode du diagnostic de la récidive, la taille de la récidive, l'étendue de la lésion au moment de la récidive, l'intervalle entre la chirurgie initiale et la récidive ainsi que l'histologie de la récidive.
