To the Editor: The letter from Ferreira and Grundy (May 23 issue)* describing an apparently dramatic remission of acute mountain sickness after treatment with dexamethasone should be placed in a broader perspective. Dexamethasone has been used in the treatment of acute mountain sickness in Nepal since at least 1974, usually being reserved for use in the more advanced cases, manifesting symptoms of cerebral edema. No clear-cut therapeutic response was demonstrated, and the use of the drug declined. Successful use of dexamethasone in milder cases remains to be demonstrated.
