High-Renin Hypertension in Necrotizing Vasculitis

Abstract
To the Editor: Hypertension may occur in the necrotizing vasculitis associated with drug abuse,1 , 2 but the mechanism remains undefined. We report here a fatal case of hepatitis B in which hypertension was shown to be angiotensin dependent.A 23-year-old man who had used intravenous heroin and amphetamines intermittently was found to be hypertensive (180/130 mm Hg) and hypokalemic (2.7 mmol of potassium per liter) after several months of malaise, anorexia, headaches and nocturia that resulted in a 4-kg weight loss.
