Abstract
A case of Waldenstrom''s macroglobulinemia has been presented with the rare manifestation of finely nodular pulmonary densities as a prominent part of the syndrome. These were primarily in the upper lobes and became more confluent in the perihilar and lower lobe areas. A biopsy of the lung revealed round cell infiltration of the inter-alveolar spaces, primarily with plasmacytoid cells. The intra-alveolar spaces were filled with eosinophilic material which was strongly positive to periodic-acid-Schiff stain. Chlorambucil appears to be a promising therapeutic agent and probably should be given continuously as maintenance therapy. Keywords
