Abstract
In 78-year-old woman presented with hematuria, azotemia, and anemia. Shortly after admission, she developed hemoptysis and died 24 hours later. At autopsy, the lungs showed hemorrhage, edema, and organization, while her kidneys revealed acute, subacute and chronic glomerular changes without significant vascular disease. Arteritis was not present in any organ examined. Goodpasture''s syndrome represents a distinct clinical entity character-ized by pulmonary hemorrhage with consequent iron deficiency anemia, glomerulonephritis leading to subsequent renal failure, and an absence of arteritis at autopsy. Most patients do not respond to steroid therapy. Eighty-nine males and 16 females were found in the world''s literature who fulfilled these criteria. The age, sex, late onset of pulmonary hemorrhage, and subsequent rapid death in this 17th female patient are unusual features of Goodpasture''s syndrome.
