Hypercalcemia and Hypertension

Abstract
Three consecutive patients with hypercalcemia associated with reversible hypertension are presented. This association has not received much stress in the past. The fact that many case reports neglect to mention blood pressure makes the actual incidence of the association difficult to determine. One-third of hypercalcemic patients may have significant hypertension. The mechanism is not well understood; suggestions have primarily centered around a renal origin. Recent experimental evidence supports a direct effect of Ca upon peripheral vascular tonicity and the infusion of Ca in tests for hyperparathyroidism has been reported to result in significant elevation of blood pressure in a large percentage of patients. This hypertension is promptly reversible on cessation of the infusion. Hypertension should be considered a common additional clue to hypercalcemia, and hypercalcemia should be added to the differential diagnosis of hypertension which may be correctable.
