Granulomatous Myositis and Myopathy Associated with Crohn's Colitis

Abstract
Extraintestinal complications of inflammatory bowel disease are well recognized, but extraintestinal primary Crohn's lesions are less common. Unusual bone1, synovial2 and vulvar3 granulomatous involvement has recently been described. Granulomatous myositis and myopathy may also accompany Crohn's colitis.Case ReportA 44-year-old man was admitted to the hospital with a six-week history of non-bloody diarrhea, crampy abdominal pain, arthralgia, fever and a 4.5-kg weight loss. Physical examination was unremarkable. Proctoscopy was normal to a level of 20 cm from the anal margin, at which point multiple ulcerations, 2 to 3 mm, extended to the length of the proctoscope.
