Abstract
Congestive heart failure remains one of the most prevalent and costly syndromes to treat throughout the world. In an era when cost-effective, quality, outcome-based care is a mandate, advanced practice nurse managed heart failure clinics propose a solution to this formidable problem. This paper examines a model of care based on self-monitoring utilized by an advanced practice nurse to effect the inter-related outcomes of readmission and cost of care.Keywords
