THE RELATION OF HIGH AND LOW UREA CLEARANCES TO THE INULIN AND CREATININE CLEARANCES IN CHILDREN WITH THE NEPHROTIC SYNDROME

Abstract
In a group of 33 children less than 10 yrs. of age with the nephrotic syndrome, the urea clearance in 14 was elevated to 140% or more of the normal range for periods of 1 or more mos. In 3 children exhibiting this "high-clearance" phenomenon, the simultaneous clearances of inulin, and exogenous creatinine were elevated above normal to approx. the same degree. The calculated tubular excretion of creatinine was found as much accelerated a,s was glomerular filtration, as indicated by the inulin clearance. Simultaneous urea, inulin, and creatinine clearances were also performed on 2 children who had recovered from acute hemorrhagic Bright''s disease, and on 2 children and 1 adult with the nephrotic syndrome and depressed urea clearances. The intraven. adm. of a prep. of amino acids obtained by the enzymatic hydrolysis of casein did not increase the clearances in patients with diminished renal function.
