Abstract
A patient with progressive pulmonary, mediastinal and peripheral lymph node sarcoidosis developed evidence of respiratory insufficiency. Diagnosis was established by femoral lymph node biopsy. Tuberculosis, etc. was ruled out by adequate investigation. Acthar-C (ACTH) derived from sheep pituitary was administered. Opt. dosage was 80 mg. intramusc. or 25 mg. by slow intraven. infusion. Circulatory eosinophiles dropped from 450/cu. mm. to 11/cu. mm., 17-ketosteroids increased from 4.5 mg. to 25 mg. per 24 hrs. Mild hypopotassemia was corrected with KC1. Marked improvement occurred clinically, roentgenographically and pathologically. The findings suggest that progressive sarcoidosis responds favorably to intensive and prolonged admn. of Acthar.C.Keywords
